Special Education Case Manager

Duties

The Case Manager ensures that students with IEPs and 504 Plans receive the necessary accommodations, modifications, and supports as outlined in their plans. This position involves collaboration with staff, consultation with families, monitoring student progress, and ensuring compliance with legal and program requirements. Regular communication with teachers and support staff is essential, providing them with quick summaries of student needs and offering ongoing guidance and problem-solving. To monitor the effectiveness of these supports, classroom observations are conducted, and any learning or behavioral concerns are documented. Serving as a central point of contact for families, addressing their questions, and collaborating on solutions is also a priority, along with communicating with the student's school-year team for consistency. Detailed record-keeping is crucial, including logging provided accommodations, communication with stakeholders, transportation arrangements, and culminating in an end-of-year report summarizing each student's progress and future recommendations. Logistical coordination ensures students have the necessary transportation and that any barriers to their participation are addressed, including monitoring the support provided by additional aides.
