Contact Intermediate Service Center 4 (ISC4) at 708-754-6600 to start the process.
Once you have all the required forms including the authorization certification from ISC4, Call our HR Department at 708-423-0950 x2116 to request a substitute application packet to be picked up at the District Office located 2929 W. 87th Street between the hours of 9:00 a.m. - 3:00 p.m.
